eligibility

IMA offers valuable benefits to eligible associates and their family members:

  • Full time associate: Working 30 or more hours per week
  • Part time or temporary: Working 20 or more hours per week
  • Spouse: Opposite sex, same sex, common law marriage, civil union or domestic partner
  • Dependents: Up to the age of 26, regardless of employment, marital, student or financial dependency status; any child of any age who is disabled and incapable of self-support

making changes

The choices you make when you first become eligible are in effect for the remainder of the plan year. It’s important to review your benefit options and choose the best coverage for you and your family.

You have three opportunities to enroll in or make changes to your benefits:

  1. Within 30 days of your hire date
  2. During the annual open enrollment period
  3. Within 30 days of a Qualified Life Event Change (60 days for a change under Medicaid or CHIP)

Keep in mind that you may be required to provide supporting documentation, such as a birth certificate or marriage certificate, to prove your dependent’s eligibility.

Qualified Life Events (QLEs) are major life changes that can affect your coverage needs.

A few examples of QLEs include:

  • Marriage, divorce or legal separation
  • Birth or adoption of a child
  • Death of a dependent
  • Loss or gain of other health coverage for you and/or dependents
  • Change in employment status
  • Change in Medicaid/Medicare eligibility for you or a dependent
  • Receipt of a Qualified Medical Child Support Order
  • HIPAA Special Enrollment Rights
