Workers’ Compensation Designated Medical Provider List

Workers’ compensation insurance coverage is provided by IMA and can help to protect you and IMA after a work-related injury or illness. Workers’ compensation provides medical care, lost wages and helps you recover and return to work safely. 

If you need to seek emergency medical assistance, please go to the nearest clinic or hospital to do so. If your injury does not require emergency assistance, please review the list of providers at the end of this document.

As soon as you are able, no later than the next day, report the incident to the appropriate Workers’ Compensation Insurance Carrier. Claim filing instructions are below.  In addition to filing your claim with the appropriate carrier please notify HRLeavemgmt@imacorp.com as soon as possible.

How to file a claim

• Associates that are not located in CO, OH, WA, ND, or WY
  • Claims are managed by Travelers Insurance, Policy Number UB-0K349422-22-14-G.
  • To file a claim please call 1-800-238-6225. Follow the voice prompts. 0K349422 is the policy number you will use to start the claim.
• Associates located in Colorado
  • Claims are managed by Pinnacol Assurance, Policy number 4111368.
  • To file a claim please call 303.361.4000 or Toll-free: 800.873.7242. Follow the voice prompts. 4111368 is the policy number you will use to start the claim.
• Associates located in North Dakota, Ohio, Washington, and Wyoming
  • North Dakota, Ohio, Washington, and Wyoming have state sponsored workers’ compensation programs. To file a claim please review the relevant state information below.

When filing a claim please have the below information readily available:

• Date and time of injury
• Specific Location of injury (address where incident occurred, as well as home office designation)
• Body part(s) injured. Be specific. Left, right, top, middle, lower are all important
• Was medical attention sought? Where?
• Was an ambulance called?
• Will there be lost time?
• What caused the injury?
• Were there witnesses? Names and phone numbers of up to two people

Understand your responsibilities

During the claim process, it is critical that you keep your medical appointments and communicate with IMA’s corporate leave and disability case manager and your workers’ comp claim manager, so they are aware of your recovery status.  Progress reports from your medical provider should be sent to HRLeaveMgmt@imacorp.com so we are apprised of restrictions or issues that may impact on your ability to do your regular duties.

It is important that you attend all medical appointments and adhere to prescription use.  If you are unable to attend an appointment, please contact the medical provider to cancel and reschedule.  You must also notify your claim manager of the appointment’s cancelation and rescheduled date.

When your medical provider releases you back to work, you will need to notify HRLeaveMgmt@imacorp.com and your claim manager prior to your return.  If you’ve been cleared to return to modified duties, your manager may be able to make accommodations for any restrictions resulting from your injury/illness or provide you with an alternative duty assignment.

If an associate is injured at work, during a volunteer event, or at an IMA-sponsored event, the employee, manager, and any witnesses must complete Sections 2, 3, and 4 of the Investigation Form and submit it to HRleavemgmt@imacorp.com. Human Resources will complete Section 1.

Medical care

To provide the best medical care for our associates at a reasonable cost in the event of an on-the-job injury, IMA has designated the following locations to provide all medical services for any employee injured on the job. Unless you reside in one of the monopolistic states (Ohio, North Dakota, Washington, Wyoming), you must seek treatment from one of the designated providers listed in your area. If you go outside of the workers’ compensation network your expenses incurred may not be covered under the workers’ compensation claim.

MEDICAL BENEFITS: An employer is required to furnish all necessary medical treatment and has the right to designate the treating physician. If the employee seeks treatment from a doctor not authorized by the employer, the employer or its insurance carrier is only liable for up to $500.00. Management retains the right to change this policy without prior notification or consent of the associates.  Supervisory staff may use discretion in the treatment of emergency cases or injuries.

If you reside in California:

California State Certified Medical Provider Network (MPN)

Unless the employee has pre-designated a physician prior to the injury, the employee must choose a provider from within the approved MPN.

Kaiser specialists are available by referral only from a Kaiser Permanente On-the-Job (KPOJ) Primary Treating Physician.
